Pasta or Bunot Ngipin and SSS


Day 1. Sunday Feb 23, 2014. Attempt.

My tooth is aching. Its not that bad. its just there's a hole and every time that a food gets into it, i struggled to take it away, then thats the time of having pain. So i decided to have a visit to the dentist.

I have this card given to me by my previous company. I have checked their booklet and it says that i have a free filling.

 


  
i was hesitant (because usually i rather pay to avoid a lot delay, less priority etc etc). But then, i gave it a try and visited Unihealth in Tagaytay. Kindly search of Google where is it located. A call center from Asian Life referred me to Doctora Penafrancia Simon. She's available on Sunday from 1-4pm..

I arrived at 2pm but the Doctor told me to go back tomorrow. a lot of patient in line...

Day 2. Monday. Feb 24, 2014. Finally.
Since the doctor told me to go back. This time i arrived early about 1pm. Good thing i saw Aleli from UPLI and told me to confirm my card on HMO which is also located on the 2nd floor of Unihealth. The dentist is located on the 2nd floor right side.

1. Went to HMO. told me to ask directly on the dentist.
2. Gave the card to the dentist and waited.
3. waited for my turn. The dentist didn't made me fill up the form. she has no secretary. she personally filled up the form from my answers to her questions.
4. done with my pasta
5. she said, if the pain continues, no choice but to take it away, bunot! ouch. she gave me a calling card and told me to text her first or have an appointment with her.

SSS Inquiry
















Inquiry about my Change of status, from Employed to Voluntarily

1. go to Information (most right side)

2. fill up form (they gave me the RS-5 form) since they informed me no need to change, just check the Type of Payor.
fill up also the Verification Slip (for you to ask question)
ask for your number
** note: bring ballpen!


3. the Customer Care (most left/Generalist) informed me that i don't need to pay the months of no contribution since i was unemployed from April 2013 to Dec 2013 But! i need to pay 1650 starting January 2014 MONTHLY!!! OMG.

Anyway, i did not pay yet, because i have only 2k.

**note. for you to have SSS Unified ID, need to have 3 months contribution.
The form will be available on the time that you are applying for the ID. First 100 to be served only.
