Pasta or Bunot Ngipin and SSS


Day 1. Sunday Feb 23, 2014. Attempt.

My tooth is aching. Its not that bad. its just there's a hole and every time that a food gets into it, i struggled to take it away, then thats the time of having pain. So i decided to have a visit to the dentist.

I have this card given to me by my previous company. I have checked their booklet and it says that i have a free filling.

 


  
i was hesitant (because usually i rather pay to avoid a lot delay, less priority etc etc). But then, i gave it a try and visited Unihealth in Tagaytay. Kindly search of Google where is it located. A call center from Asian Life referred me to Doctora Penafrancia Simon. She's available on Sunday from 1-4pm..

at Unihealth.
I arrived at 2pm but the Doctor told me to go back tomorrow. a lot of patient in line...

Day 2. Monday. Feb 24, 2014. Finally.
Since the doctor told me to go back. This time i arrived early about 1pm. Good thing i saw Aleli from UPLI and told me to confirm my card on HMO which is also located on the 2nd floor of Unihealth. The dentist is located on the 2nd floor right side.

1. Went to HMO. told me to ask directly on the dentist.
2. Gave the card to the dentist and waited.
3. waited for my turn. The dentist didn't made me fill up the form. she has no secretary. she personally filled up the form from my answers to her questions.
4. done with my pasta
5. she said, if the pain continues, no choice but to take it away, bunot! ouch. she gave me a calling card and told me to text her first or have an appointment with her.

Inquiry about my Change of status, from Employed to Voluntarily

1. go to Information (most right side)

2. fill up form (they gave me the RS-5 form) since they informed me no need to change, just check the Type of Payor.
fill up also the Verification Slip (for you to ask question)
ask for your number
** note: bring ballpen!


3. the Customer Care (most left/Generalist) informed me that i don't need to pay the months of no contribution since i was unemployed from April 2013 to Dec 2013 But! i need to pay 1650 starting January 2014 MONTHLY!!! OMG.

Anyway, i did not pay yet, because i have only 2k.

**note. for you to have SSS Unified ID, need to have 3 months contribution.
The form will be available on the time that you are applying for the ID. First 100 to be served only.

Philhealth Trece


Since i am officially unemployed, i have to change my status from EMPLOYED to VOLUNTARY OR SELF EMPLOYED to continue my payments for Philheath since mom will need this for her eye operation.

Note: My last contribution was last Feb "2013" and Philhealth informed me that i have to start my payment again.

So i will be paying "2014". From 400 per month as employed down to 200 per month as voluntary paying citizen.


Location: Philheath Trece
Landmark: after Waltermart trece & steel bridge. Left side besides POEA.

How to get there:
1. From Mendez to Indang - thru jeep paid p15/16

2. Indang bayan. ride a jeep with Dasma Bayan sign board. Tell the driver Philheath Trece. paid P15.




In the Philheath Vicinity:
1. log your name, purpose, signature on the guard's Log book
 

2. approach the Customer Service. explain your purpose of visit
Since i have to change my status from Employed to Voluntary they gave me:

 - the AF form or Amendment Form (for status change)
 - Philhealth Premium Payment Slip since i will update my payment
 - Transaction Number. Unfortunately im 141 queue.
 

3. fill up the information needed. Wait for your turn.
good thing there are seats available, with TV. check your turn on the LCD on the walls.

4. once its your turn, your transaction number and your counter number will appear on the LCD

5. provide the form to the officer. you can ask questions and they will accommodate you. me, i just waited him to change my status on the computer. He handover me my printed updated Members Data Form. 

He then advised me to go back to Customer Service and ask for a transaction number for Cashier payment

6. again wait for your turn. Provide the form to the Cashier.
Since i didnt want to go back again to pay my contribution. I paid:
Jan-March= 600 (200 per month) and
April-June=600.
Total of 1200.

note: you can pay quarterly, semi-annual or annual based on what you desire.

7. That's it! im done. i just have to take care of the papers since this will be needed in due time.





Since its almost lunch time. as usual i need to eat. Ate, Yani and I decided to eat at Waltermart Trece which is nearer. We rode a jeep and paid 8 pesos.

Its been so long since I rode the bus and its boring so what to do? SELFIE as usual. haha
Excuse for the face. No sleep yet but there is no difference if with sleep. :)

Reality of Life

  

Some of the photos i took using my cellphone. It may not be a professional shot but its about people you normally see everyday but once you get to look it deeper side of it, you will notice, it has an impact in your life. You will realize how blessed you are. or you can get something to learn from them.

A tragic disaster


 During lunch, i went to Robinsons Pala-pala. I saw this young kids. Smelling rugby. Its so sad. They say that they do that so they wont get hungry.


 I was inside the bus when i took this photo. It was too hot outside but this woman carrying her baby while selling candies & cigarettes. I asked myself, how much income or interest they will get just by selling P1 candy.


I was inside the jeep when i saw this near Robinsons Pala-pala. A kid carrying a baby and trying to get in the jeep just to ask for alms. Poor baby.


I saw this kid in Dasma eating rice without utensils. Since i have extra money, i ask him to join me for lunch. I ask him to order and i will pay the bill. When i thought he will join me on the table to eat, he said it was for take out because he will give the food to his father.


 An old woman on her wheelchair asking for alms outside the church is Dasma Bayan.

A cute female dog who was so scared and went inside the guard post in our office in Dasma. She went away. I hope someone took care of her or she find her way home. :(

I always ask myself whenever i see this. Is it really proper to preach for God's word then ask for donations?

When loving someone becomes too much


its a sensitive topic but it affects me just now.


its so sad. seeing status in facebook.

how can i guy do something just to bring his ex girlfriend down?

i know some of you will say, its the girl's fault. she trusted him. she became "malandi" or "easy to get" you can say everything but we cannot judge her until you are in her place.

the story was. once upon a time, there's a girl and a boy fell inlove with each other. everything is ok at first but sometimes not everything ends with a happy ending. they broke up.

the girl did the break up.

and the boy.. do the nastiest thing. he posted the intimate photos of the girl and threats that he will post the sex videos of them.

ok, i know you will say, its the girl's fault because she agreed doing that but hey, be open minded. i dont say here that it is acceptable but we still cant judge.


my concern was, how can a guy do that to her. they shared happiness. they shared moments. not everything is bad. the person you are trying to bring down was the person who you loved before.

what's the purpose of doing that? to reveal how bitch she was? the girl dont pretend that she is a saint.

why put it in social sites where everyone can see? is it the proper way to get even?

i know, we cant deny that she has those photos and videos. but the fact that she trusted  him with all her soul, how can he break her into pieces...

It was Friday night that we decided to go out and try Wensha Spa because of he is getting too much stress from work.


Woke up 7am. i don't want to leave my bed because it was way too cold to go out my room. had a peak on my door to see whats outside and i saw my cutie baby nephew playing around while dad is watching over. so i decided to go out of my room.

had only a chocolate hot drink for breakfast then played with my nephew. seem he was sleepy already and i decided to carry him and his head fall on my shoulder. such a wonderful feeling that he trusted me.. he spoke words which i jokingly told mom his ritual then he falls alseep already. :)

anyway, i need to rush because our target meet up in Baclaran was lunch time.


I rode the slow San Agustin bus at 8:30am and payed 84 pesos for Mendez-Coastal Mall.
Then walk in footbridge and rode again the jeep for 8 pesos for Coastal-Baclaran.

 (Offtopic: so whats the improvement of not letting the provincial buses from entering   Manila? it was still a fricking traffic! walking was not the big deal here for they are saying that Filipinos are "tamad"/slack. The point is:
old fare: Mendez- Baclaran = 87 pesos
now: Mendez-Coastal Mall 84; Coastal-Baclaran 8 = 92)




We met in front of our Baclaran church and rode a taxi. Actually i was hesitant getting a taxi because of the news i heard involving them but he insist since he said that we will going to walk too far.
From Baclaran-Wensha (near World Trade/Boom na Boom) = 69 pesos (with flagrate of 40)

I regret that i didn't got the plate number of the taxi because of manong driver was so nice to us. he didn't asked for additional payment or tip or didn't even insist us to pay this amount.

(not like the taxi driver i had last time i went to alabang. i gave him 50 pesos due to the rate indicated in the machine, he insist me to pay 100 from Festival Mall to Alabang Town Center and locked the door. i was scared and angry and threw the 100 pesos on his face and hurridly left the taxi)

We are here! 12:16pm and its lunch time. I really decided to eat there to avail the buffet.
Paid 780 for the Whole Body Massage


The cashier gave me the bill and rubber bracelet with the locker key and number. I wont go on to details

1. surrender shoes, show them your number
2. provide the number in the lady in the locker room (boys and girls room are separated)
she will give you the rob and a towel
note: there is no dressing room! you will undress inside the locker room.
that's the reason behind that boys and girls are separated because you are going to see undressing and naked people
luckily my locker was on the lower layer because others need a ladder to climb just to get to their locker
the bracelet key is steal/magnetic/infrared. you have to tap it and there's a red light open you open it
3. since its lunch time, we meet on the dining area.
we had rice and different viands which are all ok.
unli drinks. unli ice cream. shabu-shabu
4. went back to girl's locker room.
5. took a shower (hot/cold of your choice) with shampoo/conditional and body wash
6. headed to sauna (all are NAKED from here)
7. then to steam sauna with cube ice (i have to research whats the purpose with putting ice on your face inside the sauna)

8. jacuzzi (hot/cold) - i was not interested
9. got tired, went out on the locker room then passed dinning area and headed to massage room
10. waited for my turn, open room so again you will see other people barely naked, part of their bodies are covered by towel
11. choice of mineral or ginger oil (i had ginger oil)
12. massage itself
13. you can say yes or no for stretching (i tried, feels like i was in wrestling game)
14. choice to sleep for 30 mins (i am not sleepy)
15. signed a paper and indicate how much is your tip (hmmm..)
16. back again to dining just to eat again. hahaha. remember everything is UNLIMITED but you have to eat everything. no leftover. be realistic.
17. want to go home

18. dressed up inside the locker
19. free blower or hair iron of your choice
20. free toothbrush, toothpaste, a plastic cup to use
21. free eskinol and cotton (i didnt used) only the cotton buds
22. free johnsons facepowder and lotion
23. i didnt put a tip haha
24. returned the bracelet key
25. paid the tip (based on what i said on the paper awhile ago)
26. they brought my shoes

NOEL BAZAAR

It was raining outside. Good thing i brought umbrella. we are planning to get a taxi outside wensha but we saw the World Trade Center just in front of us so we decided to walk until there.


1. went in the Bazaar
2. paid 50 pesos entrance
3. a lot of stalls/booth more on those who are selling clothes
4. with free show
5. there are also selling food
6. nothing much for me
7. went out
8. plan to get a taxi (its been difficult) so we just walk
9. luckily almost empty minicab going to SM MOA
10. paid 8 pesos

SM MALL OF ASIAso tired to roam around the mall. i just decided to go here to get a van. super long waiting people for Dasma Bayan Van so i decided to get the Imus BDO Van which cost 50 pesos. they say its too costly.

IMUS BDO. luckily an Tagaytay-Mendez Erjohn Bus came! paid 66  pesos! Homebound!

Hi! Its my first time to create a blog. When i was a kid, i used to write and tell stories on my diary. i also love listing the things that happened to me on a specific day which i want to remember.

Anyway, i'll just make a draft about my 1st experience in Wensha Spa Pasay yesterday dated Nov 23, 2013 and also my 1st time in Noel Bazaar at World Trade Center :)
